An abnormal BP response to exercise is best appreciated by an understanding of the Angina Pectoris response. The systolic pressure normally rises and the diastolic pressure normally stays the same or falls during exercise. In patients with significant myocardial dysfunction, the systolic blood pressure may rise, but then falls with continued exercise. In patients with hypertension, there is often an exaggerated systolic rise and no diastolic fall in blood pressure.
