The apical impulse is non displaced and sustained systolic. A sustained apical impulse typically reflects a pressure, or afterloaded, left ventricle, as seen in patients with longstanding systemic hypertension. In the video, note the sustained contour of the precordial impulse, as illustrated by the cottons swab. The cotton swab over the carotid artery identifies ventricular systole.
