A congenital heart defect, such as an uncomplicated left-to-right shunt, is suggested by identification of a heart murmur in early infancy and no other symptoms of cardiac abnormality developing over time. Presence of a family history positive for a congenital heart defect is important, as is the absence of both a recent illness and significant cardiac symptoms.
